Briefly describe establishment’s production/ operation technology with an illustrating map indicating waste current location and/or locations probably causing environmental problems not because of wastes (if any).
1.7. Machinery and equipment
List all machines and devices/equipment which were installed and are in operation; is being installed (for establishments prescribed in Clause 1 Article 15 in this Circular) with detailed instructions about name, place of manufacture, year of manufacture, operating status (brand-new or secondhand; if secondhand, how new it remains)
1.8. Materials and fuel
List all materials, fuel and chemicals needed with detailed instructions about business name, chemical formula (if any), and quantity needed in time units (date, month, year)
Clearly indicate the needed quantity of power, water and other materials in time units (date, month, year)

- Summarize establishment’s environment protection status in their operation process.
- Reasons for not building up an environment standard registration, an environment protection commitment or an environment protection plan before.

Requirements: In case the establishment was already handled, copy and attach the punishment documents in the appendix of this project.
CHAPTER 2. THE DESCRIPTION OF WASTE SOURCES AND ESTABLISHMENT’S ENVIRONMENT IMPACTS, ENVIRONMENT PROTECTION CONSTRUCTIONS AND MEASURES
2.1. Waste generating sources

Requirements:
Describe in details each of waste generating sources together with specific calculations about: each establishment typical parameter’s levels with respective technical regulations; total amount of each typical parameter (kg, tone, m3) and that of the entire source in one day and night (24 hours), one month one quarter of a year and one year. If there are two different emission places and above, calculate the total emission amount of each place.
2.1.5. Source of noise and vibration
Describe all sources of noise and vibration
2.2. Socio-economic and environmental impacts
- Describe all environment problems caused by the establishment (if any) such as erosion slide, falling, sparing; deposition in river/ stream/ lake bottoms; surface water level changes, underground water level; salt/ alum marsh; climate changes; the environmental component recession; biodiversity transformation, and other factors.
All contents in index 2.1 and 2.2 shall be clearly indicated in each stage as follows:

2.3.1. System of collecting and treating wastewater and rain-water
2.3.2. Vehicle and devices to collect, store and treat ordinary solid wastes and hazardous wastes

2.3.4. Measures to avoid noise and vibration
2.3.5. Constructions and measures to prevent and respond to the environment incidents
2.3.6. Other environment protection constructions and measures
All contents in appendices f-rom 2.3.1 to 2.3.6 shall be clearly indicated:
- Materials, fuel and chemicals (if any) have been used for construction operation, environment protection measures together with detailed instructions about business name, chemical formula (if any), quantity needed in time units (date, month, quarter, year)
- Technical procedure, operation managing procedure of waste treatment constructions, treatment effectiveness and comparison of the results with applicable environmental technical regulations.
- If waste treatment is hired to be done, clearly indicate the hired facility’s name and address together with the hiring waste treatment contract (copy and attach the document in project appendices)
- Assess the effectiveness of prevention and socio-economic impact reduction measures and compare with applicable laws.
2.4. Plans of building, improving and operating constructions and implementing environment protection measures.
For establishments with unfinished constructions and environment protection measures non-compliant with environmental standards
CHAPTER 3. ENVIRONMENTAL MANAGEMENT PLANS
3.1. Reducing adverse impacts by wastes
- Respective measures with presentation of practicality level and solving effectiveness. In case there are no solutions or there are but impractical in the project scope, reasons shall be clearly stated and specific proposals shall be given so that relevant authorities direct their solutions and decisions.
- Prove to what extent the waste solving will be processed after applying the measures; compare and contrast with the standards, norms and applicable rules. In case it cannot meet the regulated requirements, reasons shall be clearly stated and specific proposals shall be given so that relevant authorities direct their solutions and decisions.
3.2. Reduce other adverse impacts
Each generating waste type shall be accompanied by the respective solutions and practicality/solving effectiveness presentation. In case there are no solutions or there are but impractical in the project scope, reasons shall be clearly stated and specific proposals shall be given so that relevant authorities direct their solutions and decisions.
3.3. Plans to supervise the environment
- Need to supervise the emission and sewage flow and establishment’s typical pollution index in emissions and sewage, compliant with applicable environmental technical regulations, with minimum frequency one (01) time/06 months. For establishments connecting with concentrated waste treatment system of concentrated production, business and service areas, do not need to supervise sewage.
- All monitoring points shall be showed in the map with clear explanation and coordinates as in applicable laws.
Requirements:
- For the case of extension, upgrade, raising production capacity, in the contents of Part III at this appendix it is to specify results of activated establishment’s implementing measures to avoid adverse impacts, prevent and respond to the environmental incidents and analyze their reasons.
- For establishments re-build up environment protection project, in contents of Part III at this appendix it is to clearly specify all changes in measures to avoid adverse impacts, prevent and respond to the environmental incidents.
- Apart f-rom describing the solutions to reduce adverse environment impacts as instructed in Point 3.1 and 3.2 at this appendix, constructive solutions to reduce adverse environment impacts shall be listed in tables in which all types, technological c-haracteristics, necessary measurements are clearly indicated with specific progress for each project.
- For the case of extension, upgrade, raising production capacity, contents of Part III at this appendix need clear specification of the constructions’ quo status, current solutions to protect the environment of active establishments and the relation between these constructions and the construction system, solutions to protect the environment of extension, upgrade, raising production capacity projects.
CONCLUSION, RECOMMENDATIONS AND COMMITMENT

- Ability to identify, describe all waste sources and calculate waste types, identify and describe all environmental, socio-economic problems;
- Effectiveness and practicality of environment protection constructions and measures; solve environmental and socio-economic problems generated f-rom establishment’s activities.

Recommendations for central and local relevant levels, branches to solve problems beyond establishment owner’s solving ability.
3. Commitment
- Comit to seriously implement the contents, progress of building up, improving and operating constructions and environment protection measures.
- Commit to seriously implement reporting model in the project ratifying Decision.
- Commit to comply with environment regulations which is relevant to the establishment, including environmental technical regulations.
- Commit to compensate and tackle environmental pollution in case there are incidents in establishment’s operation process.
